Implementing Care Management Plus
If you think Care Management Plus might be right for your clinic, group,
or organization, please look through the links to the left and read
about how implementation works. We ask that organizations implementing
Care Management Plus commit to complete the process, which you can read
about in the Readiness Assessment. Also in the Readiness Assessment are
several questions to help you determine if you are ready to implement
Care Management Plus, and to help you nail down what your goals are.
