    • Many studies have been done on information technology systems created or enhanced to improve care for patients with chronic illness; this literature review examined over 100 such studies and determined that the systems had benefits the majority of the time (67%) but failure to adequately demonstrate that benefit was common. Specific enhancements seemed to lead to greater success, such as connection to an electronic medical record, computerized prompts, population management (including reports and feedback), specialized decision support, electronic scheduling, and personal health records.

    • We demonstrate an 8-12% improvement in productivity of providers who actively send patients to the care manager versus those who do not; this pays for the Care Management program.

    • This paper highlights the Care Management Plus implementation and compares it to previous Care Management/coordination like the Chronic Care Model. The technology, care manager and health system infrastructure, and process are described in detail.

    • For patients with diabetes, the program was very effective at improving process and outcomes measures of diabetes control. The reductions were estimated to lead to a 20-35% reduction in complications of diabetes.
